The Department of State Health Services (DSHS) is required to provide to the legislature the annual numbers of immunization exemption affidavit forms requested for students in Texas child-care facilities, elementary schools, secondary schools, or institutions of higher education during the preceding academic year per the Texas Legislature.

Texas law allows for an exemption from immunizations for reasons of conscience, including a religious belief. To claim an exemption due to reasons of conscience, a student's parent or legal guardian must request, sign, and submit an official DSHS affidavit form to the child's school. The affidavit is valid for two years from the notarization date.
