Obtaining a Certificate of Registration
For a medical or academic certificate of registration, submit the following forms with the appropriate fee:

Fee Schedule for New Certificates of Registration
The table below outlines fees pertaining to the issuance of a single certificate of registration for medical use.

Each initial application for a certificate of registration shall be accompanied by a non-refundable fee equal to the appropriate biennial fee. Fee payments by check or money order shall be made payable to Texas Department of State Health Services.
Multiple Categories of Use
In the case of a single certificate of registration that authorizes more than one category of use, the total fee is the category listed in 25 TAC, §289.204 with the highest corresponding fee. If the certificate of registration has additional authorized use locations, the registrant shall pay an additional 30% of the highest fee category for each additional use location.
Biennial Fees for X-Ray Registration
Biennial fees must be paid every two years in order to continue operating X-ray devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

Amending a Certificate of Registration
*Equipment, name, address, and RSO changes* can be completed online using the Regulatory Services Online Licensing System. For instructions, visit this page.
To amend a medical or academic registration, submit RC 226-2.

Radiation Safety Officer Change - Complete form RC-42-R.
Technical Renewal of Certificate of Registration
Renewal applications shall be submitted every eight years. The agency will send a renewal reminder letter and application by mail 45 days prior to the expiration date.
To renew a Certificate of Registration, complete and submit the following forms:

Submit one application form, RC 226-2 for each use location. Report any special authorizations such as mobile or self-referred screening on the application.
Terminating a Certificate of Registration
To terminate the registration, complete and submit RC-17-R Termination Form.
Disposing of an X-Ray Unit

Title 25 of the Texas Administrative Code (TAC) §289.227 and §289.229 requires each registrant to have and implement written operating and safety procedures. The following Regulatory Guides can be used to help assist registrants in developing procedures unique to individual facilities. For mobile services, include specific procedures unique to your mobile operation. Choose the appropriate guide:

Registration of an Existing Accelerator Due to Ownership Change

Submit RC 226-2 Registration Machine Application Amendment and include the following information:
- Supervising Radiation Oncologist credentials to include curriculum vitae (CV), American Board of Radiology Certification and Texas Medical license
- Written operation and safety procedures – See regulatory guide 4.6
- Copy of most current accelerator survey to include diagram of vault and survey summary
- Diagram of vault with shielding
- Diagram of vault with surrounding areas shown (i.e. vault, hallways, offices, parking lot)

Registration of a Replacement Accelerator Installed in the Same Orientation, Same Energy or Less, and No Additional Shielding
Submit a letter signed by the RSO that states the following information:- That the new/replacement accelerator has been installed in the same orientation, has the same energy or less than the previous accelerator, and no additional shielding is required,
- The manufacturer, model and serial numbers of the machine(s), and
- A copy of the post-installation survey
